create a directory under your baseq3 folder called models, under that new directory create a new directory that will be called players and after that create another directory that will have the same name as the custome model. Example:
the name of the folder must be the same name as the custom model, but of course you don't add the extension of the pk3 file. The explanation of this is also given in some read me file that come with custom models, here's a copy/paste from the readme from a custom model called "dragoon"

Dragoon model issues - Sgt. Science
--------------------It has been found that dragoon frequently does NOT appear on the model select screen after placing the
md3-dragoon.pk3 into the baseq3 folder, even on MY system. I am currently trying to FIX this problem.
In the meantime however, there are 2 solutions.
The FIRST, EASIEST, and most PERMENANT solution is to simply CREATE the following directory structure off your
baseq3 folder: models\players\dragoon
e.g. on my system the entire folder address looks like this:
D:\quake3\baseq3\models\players\dragoon
This folder DOES NOT need to contain ANY files. It's mere existance will jump-start the reading of information in
md3-dragoon.pk3 so that the model is visible in the model select screen.
The SECOND solution works on a per use basis. JUST because the model is not visible in the select screen does not
mean that IT DOES NOT EXIST. The following console command can load the model:
/model dragoon - loads dragoon model with DEFAULT skin by Crash
/model dragoon/bluebastard - loads dragoon model with BLUEBASTARD skin by Firestarter
/model dragoon/redbaron - loads dragoon model with Redbaron skin by Crash
/model dragoon/blue - loads dragoon model with CTF BLUE skin by Sgt. Science & Firestarter
/model dragoon/red - loads dragoon model with CTF RED skin by Sgt. Science & Firestarter
Even if Dragoon is not visible in the model select screen, if the pk3 is properly installed in the baseq3 folder,
you will still be able to view the model if other players are using it, load dragoon model bots, and load it
manualy through the console.
I apologise for the inconvienience, and hope to find a solution that works WITHOUT making you mess around soon.
Thank you
Sgt. Science

Hope this help you!"Everybody knows Rocket Launchers have proximity safety sensors that lockdown the firing mechanism to prevent point blank use, not my fault, stupid game."
