Ive been having this problem with my Logitech Mouseman Wheel Optical for years. It seems that the 4th mouse button(thumb button) never works or is no bindable in-game. The wheel doesnt work either, even though I can bind it to prev and next weapon in the menu. I am running WinXP Pro (it didnt work with 98se or 2000 pro either when I had those OSes), and I have tried installing the Logitech mouseware (all versions) but that doesnt work either. Does anyone else have these problems? Or even better, a solution that you might have stumbled across. -Mike

I assume that you are using the drivers of logitek. In my case I've got a microsoft mouse with 5 buttons and before that I used to have a logitek also. I had the same problem. The only thing that I could do to make the fourth button work in quake 3 is to change the drivers. Use the generic drivers and not the one from logitek and your fourth button should work. It did for me and one of my friend also did the same thing. The drawback. You don't use the logitek drivers.. :P After a while it didn't bother me anymore since I prefer to bind my favorites weapons on my mouse..

What I found with my 5 button INtellimouse is to make the 4th and 5th button work I have to go into the mouseware in Windows and bind them to something like F5 and F10, Then you can go to the setup in Q3 and bind whatever you want to F5 and F10 and it'll work with those buttons. For the mousewheel to work on weapon next/previous I've found it's best to just leave it alone and it'll work. But...once you go into the setup and change anything at all, the wheel will stop working until you exit and restart the game.
