Make sure that your ...\Quake III Arena\baseq3 contains a file called "q3config.cfg".

...and um, you can open pak0.pk3 with winzip to make sure that the default.cfg file is intact... I think... not 100% sure.

also try starting the game right from the exe file in the Quake III Arena folder. Sounds odd but I've had that happen after a fresh install and trying to start the game from the shortcuts it installed. Might not help in your case but it's worth a try.
